Onboarding Items Needed

  • Copy of a Company Check

 

  • Proof of Federal ID (any of the following will work
    • EIN letter within 3 years
    • Most recently filed 941
    • Most recently filed 1120 if applicable

 

  • Proof of State ID(s) (any of the following will work)
    • State ID confirmation letter
    • State ID registration
    • Most recently filed State quarterly return(s)

 

  • Proof of State Unemployment Tax ID (any of the following will work)
    • MESC Rate Determination Letter for current year
    • Screenshot from State website showing ID, computed rate & Obligation Assessment.

 

  • Payroll Company Setup form (attached)

 

  • Payroll Employee Setup form (attached)
    • Or similar report containing this information.

 

  • Previous Wage History (if applicable)
    • YTD summary:  January 1st through December 31st, 2017 with each employee broken down from gross to net and each tax and deduction separated
    • Summary:  January 1st through the most recently closed quarter with each employee broken down from gross to net and each tax and deduction separated
    • Payroll summary for each check date within the current quarter with each employee broken down from gross to net and each tax and deduction separated

 

Once setup has been completed there will be two documents requiring an owner/officer signature:

  1. Federal Reporting Agent Authorization
  2. State Power of Attorney(s)
